OBJECTIVE: To evaluate the impact of a multifaceted strategy for quality end-of-life care in a tertiary public hospital in Brazil. METHODOLOGY: The study design was quasi-experimental. The multifaceted strategy was applied between January and June 2017, and involved training the healthcare team in end-of-life discussions, the creation and documentation of advance directives, and consultation with the team specialized in palliative care. The periods analyzed were the pre-test period (Time 1, July 2015 to June 2016) and the post-test period (Time 2, July 2017 to June 2018). RESULTS: Time 1 involved 302 deaths, with an average hospital stay of 21 days; Time 2 involved 410 deaths, with an average hospital stay of 16 days. Patients were prescribed morphine (44.04% vs. 36.3% [p = 0.367]), methadone (9.60% vs. 4.39% [p = 0.247]), midazolam (43.05% vs. 47.80% [p = 0.73]), blood transfusions (31.13% vs. 24.63% [p = 0.828]), enteral feeding (56.62% vs. 38.54% [p = 0.59]) and antibiotic therapy (50.73% vs. 50.73% [p = 0.435]). CONCLUSION: This study found no changes in the end-of-life care quality indicators after the strategy was implemented. Multimodal educational strategies that develop communication skills in palliative care may enhance the quality of end-of-life care.

PURPOSE: To prospectively evaluate sexual function, body image, quality of life, and disease progression rates in women with advanced cancer. METHODS: Sixty women staged III-IV breast, gynaecological, or colorectal cancer completed the Female Sexual Function Index (FSFI), the Body Image Scale, the European Organisation for Research and Treatment of Cancer Quality of Life Questionnaire (EORTQL), and the Beck Depression Inventory. Forty-three women (71.67%) completed the questionnaires again approximately 1 year later. The Wilcoxon test was used to compare the scores between the two evaluations. The participants were informed that they could attend additional consultations for symptom control or management of sexual difficulties if they wished. RESULTS: Twenty-eight women showed disease progression (65.12%), and seven died. Twenty-one women remained sexually active (48.8%), all of whom were married/partnered. There was no difference in the FSFI score over time [28 (21.4-32.1) vs. 30.3 (24.2-33.3) P = 0.19]. The body image scores improved [6 (1-15) vs. 3 (0-10), P = 0.039], while the perceived global health status worsened [EORTQL: 75 (50-83.3) vs. 41.7 (25-50), P = 0.001]. CONCLUSIONS: Body image scores improved over time despite a concomitant decrease in the perceived global health status. A portion of women with recurrent or metastatic cancer remained interested in sexual intimacy. New studies are needed to understand the differences between the needs of women with advanced cancer who are vs. are not sexually active, for the development of more effective therapeutic approaches.

BACKGROUND: Previous studies have reported the safety of laparoscopic radical hysterectomy for treatment of early cervical cancer, as option to laparotomy. This study aims to compare overall survival between laparoscopic versus abdominal radical hysterectomy for early cervical cancer. METHODS: A single-center randomized controlled trial enrolled 30 patients with clinically staged IA2 cervical cancer and lymphovascular invasion, IB and IIA, who underwent laparoscopic radical hysterectomy (16) or abdominal radical hysterectomy (14). RESULT: The mean overall survival time was 74.74 months (CI 95%: 54.15-95.33) for LRH 91.67 months (CI 95%: 74.97-108.37) for ARH (log-rank test = 0.30). The mean disease-free survival time was 81.07 months (CI 95%: 60.95-101.19) for LRH and 95.82 months (CI 95%: 80.18-111.47) for ARH (log-rank test = 0.371). The overall survival hazard ratio was 2.05 (CI 95%: 0.51-8.24), and the disease-free hazard ratio was 2.13 (CI 95%: 0.39-11.7). CONCLUSION: Our study suggests a non-significant trend of worse outcomes for LRH. In light of recent controversy and need for prospective studies, further studies in different populations are required for definite conclusions and until then, patients should be aware of risks and benefits, survival data and quality of life outcomes related to both surgical techniques.

OBJECTIVE: To estimate the human resources and services needed to meet the demand of the Brazilian population who would benefit from palliative care, based on the population growth projection for 2040. METHODS: Population and mortality estimates and projections were obtained from the Brazilian Institute of Geography and Statistics. Service needs were estimated based on literature data. RESULTS: The expected increase in the Brazilian population for 2000-2040 is 31.5%. The minimum estimate of patients with palliative care needs was 662,065 in 2000 and 1,166,279 in 2040. The staff required for each hundred thousand inhabitants would increase from 1,734 to 2,282, the number of doctors needed would increase from 4,470 to 6,274, and the number of nurses from 8,586 to 11,294, for the same period. CONCLUSION: The definition of a national strategy predicting the increasing palliative care needs of the population is necessary. The expansion of the support network for chronic and non-transmissible diseases is necessary, but the training of existing human resources at all levels of attention to perform palliative actions can be a feasible alternative to minimize the suffering of the population.

RESUMO OBJETIVO: Estimar as necessidades de recursos humanos e serviços necessários para o atendimento de pacientes que se beneficiariam de cuidados paliativos a partir da projeção de crescimento da população brasileira até 2040. MÉTODO: As estimativas e projeções populacionais e de mortalidade foram obtidas nos dados do Instituto Brasileiro de Geografia e Estatística e do Sistema Único de Saúde. As estimativas de estrutura de serviços e recursos humanos foram realizadas a partir de dados da literatura. RESULTADOS: A expectativa de aumento da população brasileira entre 2000 e 2040 é de 31,5%. A estimativa mínima de pacientes com necessidades de cuidados paliativos foi de 662.065 em 2000 e é de 1.166.279 em 2040. O número de equipes necessárias para cada 100 mil habitantes passaria de 1.734 para 2.282 no mesmo período; o número de médicos foi de 4.770 para 6.274 e o número de enfermeiros necessários para atender as demandas dessa população foi de 8.586 para 11.294 nesse espaço de tempo. CONCLUSÃO: A definição de uma estratégia nacional prevendo a necessidade de cuidados crescentes da população se torna necessária. A expansão da rede de assistência a doenças crônicas e não transmissíveis se mostra necessária, porém a capacitação de recursos humanos já existentes em todos os níveis de atenção para desempenhar ações paliativas pode ser uma alternativa factível para minimizar o sofrimento da população.

OBJECTIVE: To evaluate the need for palliative care in patients with advanced Congestive Heart Failure (CHF) hospitalized in a cardiology ward. METHOD: Application of the World Health Organization Palliative Needs tool (NECPAL) with the assistant physician, patient and/or caregiver for evaluation of indication of Palliative Care (PC). RESULTS: 82 patients with a diagnosis of class III/IV Heart Failure or ejection fraction less than or equal to 40% in echocardiography of the last 12 months were included: Mean age 68 ± 20 years, 51 male patients and 31 female patients. Forty-three patients (52.4%) were married or in consensual union and ten (12%) lived alone. The death of 46 patients (56.1%) in the subsequent 12 months would not surprise their physician, and forty-five patients (55%) had palliative care indication according to the NECPAL. CONCLUSION: About half of patients hospitalized for class III/IV Heart Failure would have an indication of Palliative Care for the relief of suffering caused by the disease.

RESUMEN Objetivo: Evaluar la necesidad de Cuidados Paliativos en pacientes con Insuficiencia Cardiaca Avanzada en estancia en una enfermería cardiológica. Método: Estudio transversal con la aplicación de la Escala Necesidades Paliativas de la Organización Mundial de la Salud al médico asistente y al paciente y/o responsable, a fin de evaluar la indicación de Cuidados Paliativos en pacientes con diagnóstico de Insuficiencia Cardiaca clases III/IV o fracción de eyección menor o igual que el 40% en ecocardiografía de los últimos 12 meses. Resultados: Fueron incluidos 82 pacientes, con promedio de edad de 68 ± 20; cincuenta e un pacientes del sexo masculino y 31 del sexo femenino. Cuarenta y tres pacientes (52,4%) estaban casados o en pareja de hecho y 10 (12%) vivían solos. El fallecimiento de 46 pacientes (56,1%) en los 12 meses subsiguientes no sorprendería a su médico, y 45 pacientes (55%) tuvieron indicación de cuidados paliativos. Conclusión: Cerca de la mitad de los pacientes que fueron hospitalizados por Insuficiencia Cardiaca clase III-IV tendrían indicación de Cuidados Paliativos para el alivio del sufrimiento provocado por la enfermedad.

Método: Estudo transversal com a aplicação da Escala Necessidades Paliativas da Organização Mundial da Saúde ao médico-assistente e ao paciente e/ou responsável, para avaliar a indicação de Cuidados Paliativos em pacientes com diagnóstico de Insuficiência Cardíaca classes III/IV ou fração de ejeção menor ou igual a 40% em ecocardiografia dos últimos 12 meses. Resultados: Foram incluídos 82 pacientes. A média de idade de 68 ± 20. Cinquenta e um pacientes do sexo masculino e 31 do sexo feminino. Quarenta e três pacientes (52,4%) eram casados ou estavam em união consensual, e 10 (12%) moravam sozinhos. O óbito de 46 pacientes (56,1%) nos 12 meses subsequentes não surpreenderia o seu médico, e 45 pacientes (55%) tiveram indicação de cuidados paliativos. Conclusão: Cerca da metade dos pacientes que foram internados por Insuficiência Cardíaca classe III-IV teria indicação de Cuidados Paliativos para o alívio do sofrimento provocado pela doença.

However, in some public Brazilian hospitals, the costs of these materials prevent their routine use. Objective To compare the costs of ambulatory synthetic sling surgery with an historical series of patients submitted to Burch surgery in a Brazilian public hospital. Materials and Methods Twenty nine incontinent patients were selected to synthetic sling surgery. Demographic data were prospectively collected and also the costs of the procedure, including drugs and materials, use of surgical and recovery wards, medical staff and hospitalization. These data were compared to the costs of 29 Burch surgeries performed before the introduction of synthetic slings. Results Demographic data were similar, although median age was lower in the group submitted to Burch surgery (46.3±8.6 versus 56.2±11.3 (p<0.001)). Cost was significantly lower in patients submitted to sling in all items, except for time spent in recovery ward. Total value of 29 Burch surgeries was R$ 217.766.12, and of R$ 68.049.92 of 29 patients submitted to sling surgery (p<0.001). Conclusion Burch surgery was more expensive than ambulatory synthetic transobturator sling surgery, even when the cost of the synthetic sling was considered.

BACKGROUND: Hysterectomy dates back to 120BC and is the second most commonly performed gynecological surgery in the world. Cosmetic demands and the necessity of rapid return to work have contributed to the minimally invasive laparoscopic approach for hysterectomy. The majority of reports describe the use of three or four incisions to perform the surgery (two or three for manipulation and one for optics). METHODS: This work describes our experience with using only two ports for 11 patients who underwent video-laparoscopic hysterectomy surgery. One port was used for the optical system, and the second was used for manipulation. Early and late surgery complications, as well as the time to return to work and daily activities, were assessed. RESULTS: The mean age of the patients was 41.4 years old (range 16 to 52 years) and the mean uterine weight was 133.54 g, ranging from 35 g and 291 g. The operative time ranged from 30 to 60 minutes (average 46.4 minutes) and the hospital stay ranged between 24 and 48 hrs. No intraoperative complications occurred, and no early or late postoperative complications were recorded. Patients reported minimal pain during the first 24-48 hrs in the hospital. Patients returned to their daily activities within seven days after surgery. Clinical care follow-up continued until the 40th postoperative day. CONCLUSION: The laparoscopic hysterectomy technique with a single port for manipulation is a feasible procedure when the uterine weight is not greater than 400 mg with little postoperative pain. The patients had an early return-to-work and daily activities and a better cosmetic outcome. These preliminary data led us to make the one-operative port laparoscopic hysterectomy the procedure of choice for patients with a low uterine weight.

BACKGROUND: Non-randomised studies have suggested that the postoperative complications of (Campos LS, Limberger LF, Stein AT, Kalil AN) laparoscopic radical hysterectomy are similar to those in abdominal radical hysterectomy. However, no study evaluating postoperative pain comparing both techniques has been published thus far. Our objective was to compare pain intensity and other perioperative outcomes between laparoscopic radical hysterectomy (LRH) and abdominal radical hysterectomy (ARH) in early cervical cancer. METHODS: This single centre, randomised, controlled trial enrolled 30 cervical cancer patients who were clinically staged IA2 with lymph vascular invasion and IB according to the FIGO (International Federation of Gynaecology and Obstetrics) classification, and underwent LRH or ARH between late 1999 and early 2004. Postoperative pain, as measured by a 10-point numerical rate scale, was considered the primary endpoint. Postoperative pain was assessed every six hours during a patient's usual postoperative care. Perioperative outcomes were also registered. Both surgical techniques were executed by the same surgical team. Secondary outcomes included intraoperative and other postoperative surgicopathological factors and 5-year survival rates. RESULTS: IA2 patients with lymphatic vascular space invasion and IB cervical cancer patients were randomised to either the LRH group (16 patients) or the ARH group (14 patients). Four patients (25%) in the LRH group and 5 patients (36%) in the ARH group presented with transoperative or serious postoperative complications. All of the transoperative complications occurred in the LRH group. The relative risk of presenting with complications was 0.70; CI 95% (0.23-2.11); P = 0.694. LRH group mean pain score was significantly lower than ARH after 36 h of observation (P = 0.044; mean difference score: 1.42; 95% CI: 0.04-2.80). The survival results will be published elsewhere. CONCLUSIONS: LRH provided lower pain scores after 36 h of observation in this series. The perioperative and serious postoperative complications ratios were comparable between the groups. BACKGROUND: Because of the advancements in surgical techniques and laparoscopic instruments, total laparoscopic radical hysterectomy can now be performed for the treatment of uterine cervical carcinoma. We assessed the feasibility, complications, and survival rates of patients who underwent total laparoscopic radical hysterectomy with pelvic lymphadenectomy. METHODS: We retrospectively collected data from the medical charts of 29 patients who had undergone surgery between 1998 and 2008. The following data were assessed: age, staging, histological type, number of lymph nodes retrieved, parametrial measures, operative time, length of hospital stay, surgical complications, and disease-free time. RESULTS: The mean patient age was 37.07+/-10.45 years. Forty percent of the patients had previously undergone abdominal or pelvic surgeries. Mean operative time was 228.96+/-60.41 minutes, and mean retrieved lymph nodes was 16.9+/-8.12. All patients had free margins. No conversions to laparotomy were necessary. Median time until hospital dismissal was 6.5 days (range 3-38 days). Four patients had intraoperative complications: 2 lacerations of the rectum, 1 laceration of the bladder, and 1 lesion of the ureter. Three patients developed bladder or ureteral fistulas postoperatively that were successfully corrected surgically. CONCLUSION: Laparoscopic radical hysterectomy is feasible and has acceptable complications. The radicalism of the surgery must be considered, bearing in mind the parametrial measures and the number of lymph nodes retrieved.

RESUMO

Os fitoestrogênios säo substâncias encontradas nas plantas, principalmente na soja, e que possuem um efeito similar aos dos estrogênios. A incidência mais baixa de sintomas climatéricos nas populaçöes asiáticas é atribuida ao alto consumo de fitoestrogênios. Esta observaçäo epidemiológica suscitou um enorme interesse no assunto, na possibilidade de novas alternativas para o tratamento de pacientes na menopausa. O presente artigo tem como objetivo revisar os estudos mais recentes sobre o assunto, e analisar as evidências existentes sobre o emprego dos fitoestrogênios no tratamento dos sintomas climatéricos, na prevençäo e no tratamento da osteoporose e na prevençäo do câncer e das doenças cardiovasculares. O papel das isoflavonas no manejo dos sintomas climatéricos, das doenças relacionadas à menopausa e ao processo de envelhecimento ainda é incerto. Existem alguns trabalhos sugerindo que os fitoestrogênios têm uma série de efeitos fisológicos, atuando em sintomas da menopausa, lipídios e lipoproteínas, funçäo vascular, ossos e no comportamento de alguns tumores embora näo existam conclusöes definitivas a respeito.
